MAMES Conference Registration Information:
	MAMES member:
		$200.00 for first registrant $170.00 for each additional registrant
	MAMES Non-Members:
		Add $50.00 to the above for each registrant (I.e. $250.00 for first registrant, $$220.00 for each additional registrant)
Registration Fee Includes:
	Continental Breakfasts - Thursday and Friday Lunch and Dinner - Thursday Refreshment and Snack Breaks Vendor and Attendee Networking Reception Copies of all Presentation and Educational Materials Complete Attendee and Vendor Contact Listing Access to the Vendor Display Area and Reception After Dinner Entertainment
Cancellation Policy:
	There will be no refunds for cancellations made after September 17, 2008. Cancellations prior to that date will be refunded minus a $30 processing fee per registration form.
Hotel Information:
	All Conference activities will be held at the Sheraton West Des Moines. Reservations can be made by calling 1-515-223-1800. The overnight room rate is $109.00 plus applicable taxes. Please specify that you are with the Midwest Association for Medical Equipment Services (MAMES) for this rate. Deadline for hotel reservations is September 9, 2008.
